Question: Hi, my mother is a patient of migraine and spondylitis. She has been complaining for sometime about a stabbing sensation from inside, like as if with a sharp needle. Multiple neurologists diagnosed her with no specific conclusion. Now she is complaining that it feels as if something is crawling inside her skin on back of her chest. Also some irregular dark patches (near black) patch has appeared on her legs between her toe and the next finger. Its like a dark irregular mark with no visible bump outside but feels like a small bump inside and is slightly painful when pressed. It naturally subsided and now skin there is back to normal, but now a similar one has appeared on her arm. Currently she is on a blood thinner and Gebapentin 300mg + 300mg daily as recommended by a neurologist in XXXXXXX

Brief Answer:
The dark patch is a side effect of blood thinner.

Detailed Answer:
Thank you for posting your query.

I am Dr. Sudhir Kumar, Neurologist, and I would try my best to help you.

I have noted your mother's clinical details and the attached picture. Based on this, the dark patches on the skin are due to a side effect of blood thinner.

This is because of bleeding underneath the skin and is referred to as ecchymoses. It should settle down in a few days after discontinuing blood thinners. However, the decision to stop blood thinner should be taken after consulting her doctor.

I hope my answer helps. Please get back if you have any follow up queries or if you require any additional information.

Wishing you good health,

What about the feeling of something crawling under her skin. Is this also because of blood thinners. When she experienced spondylitis and she started feeling her head spinning and intense sweating for 10 to 15 mins after which it would settle down, doctor prescribed blood thinners saying it's because blood is not properly reaching brain. With so many things in here whta should be done. Can you please recommend some doctor for consulting in XXXXXXX XXXXXXX

Brief Answer:
Crawling sensation could be due to vitamin deficiency.

Detailed Answer:
Thank you for getting back.

Crawling sensation under the skin may be due to vitamin deficiency such as B12. It is not due to blood thinners. Spondylitis can be treated with physiotherapy in most cases. Reduced blood flow to the brain can be confirmed by doing tests such as carotid doppler or MRI brain (to be decided after patient's clinical examination).
