Question: Hi my son is 3 months old has had 3 abdominal surgery's, one for ostomy because was born with no anus opening, G-TUBE ,THEN has to have his stoma revised because of prolapse and obstruction... he's very gassy all the time he's always screaming crying roughly 1 an half to 2 hours after every feed, he crys irritably. He screams and screams... my wife is breastfeeding and we tried straight breastmilk for awhile, we've been doing a formula supplement called elecare and still having these pains.. he crys so much he reluxs and chokes sometimes... My wife is 29 years old with breast implants, we have a 4 year old daughter that she breastfed for 1 year no issues. But now with this lil boy we having serious issues and I'm starting to believe its something in her breastmilk that's causing this, but she said it's not that and think I'm crazy for thinking it.. but I just feel like she may only be producing foremilk and not enough hindmilk.. he is in the hospital now to get some scoping down.. if they dont find anything, can it be possible the breakmilk is causing these pains and all the gas?? Thanks for any advice..

Looks like evening colic

Detailed Answer:
Hi...Thank you for consulting in here.

This is called evening colic and is quite common in this age group. This happens when the baby sucks at the breast very fast and in eagerness to drink milk will gulp in air too. Unless the air comes out like burping or flatus this discomfort will be there.

Next time check if the baby is sucking too fast and gulping in air too.
You will be more convinced.
Usually I don't advice any medicines for this as they give only temporary relief.

The two best ways to relieve this distress is -
1. Do not put the baby in lying position after feeding till the baby burps out the swallowed air.
2. If still crying - put the baby in prone position and keep patting the back gently so that the baby passes off the flatus and gets relieved.

He does it both ways bottle or as I mentioned through his g button.. just seems that something in the breastmilk is causing OUTRAGEOUS gassines and reflux problems.. I'm asking could the breastmilk be an issue??

Keep in mind he has anatomys missing angenis of the right lung, right ear canal, horseshoe kidney.

The only thing we haven't tried is formula only without breastmilk..

Hello...no please - breast milk does not cause this.
I understand but his underlying congenital anomalies do not have any clinical relevance in this regard of evening colic please.
