Dear sir,
You mentioned you are 33 years of age. Most commonly there are two reasons one is cardiac causes and other is compression of your nerve in cervical spine. Without a physical examination, I cannot absolutely rule out a cardiac cause therefore I will suggest to get your ecg and echo done. In the meanwhile apply diclofenac gel; give hot fomentation, it will ease your tightness.
If ecg and echo are normal, I will suggest to get your MRI cervical spine and NCV done especially if you also have numbness in left upper limb.
Thanks

Hi
I had ECG maybe 8 weeks ago was normal. I had shoulder pain. This tightness been there on and off for a few weeks.
I had MRI on shoulder that said it’s tenderness around the shoulder.

Needs an MRI scan
Detailed Answer:
Hello again,
It's better to get your EMG, NCV study of the upper limb to be done, and also get an MRI scan of the shoulder to reach up to a final diagnosis.
Till then, continue with physiotherapy and hot fomentation and check your blood sugar levels.
Thanks
