Question: Hi sir I am a 37 yr old male, I have observed some changes in my bowel habit in December 2018. But that was occured without any other significant issues. But for the last one month I am suffering from anal pain that sometime radiating into buttocks, sometimes to testis and sometimes to thighs also. No blood in stools or other health problems. For the last 5 years I am maintaining a weight of 75 to 78. It is still in that range only. But i am really concerned about the anal pain. That change in bowel habit is still there. But no significant constipation and diarrhoea is not at all there. What to do and what to expect sir.

Brief Answer:
May be internal hemorrhoids or perianal abscess. Require confirmation.

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, there is no blood in stool, no constipation and diarrhea, so I think anal pain may be due to internal hemorrhoids or perianal abscess.

Please let me know do you have fever, history of injury or trauma, feeling lump around anus, difficulty in seating or walking? Kindly follow up with detailed history.

I would suggest you to consult with gastroenterologist as you may require proctoscopy and Colonoscopy investigation after detailed medical examination.

Till then for pain you can take tablet acetophenone (Tylenol) 500 mg three times a day and apply 2% lignocaine Jelly locally three times a day. These medications should be available over the counter.

Avoid hot and spicy foods. Take soft diet. Do regular exercise like brisk walking. Keep your self hydrated.

Let me know I can assist you further.

Regards,
Dr Shyam Kale.

Thank u so much for your reply sir. I don't have history of trauma or injury and fever comes normally 2 to 4 times per year. No difficulty in seating or walking. Daily I am playing badminton for one hour and doing gym workout XXXXXXX 4 days per week. Sometimes i feel that something is there around anus not all the time. The bowel change is that, today I went four times to toilet all the time small quantity of stool came out and I feel that the quantity normally comes out one time is divided and cones out un short spells. And the texture if stool is somewhat different, not loose not tight also in between that. And it is not pencil thin but sometimes it is short. Is there any chance if rectal ir colon cancer sir.

Hello,

Thank you for reply.

As per your history, you are maintaining normal weight and no any other health issue so I don't think it is due colon or rectal cancer.
