Question: Hi sir. my elder brother is suffering from low platelet count i.g 5000 . presently treat is going on XXXXXXX hospital XXXXXXX , 21 days has complete but still no improvement in platelet count remain same. patient age is 42 years . medication give as below till today. stroid IVIG Dexamethoneson Rituximap - 2 dose done Romy - 1 dose done revolate tablet is given one perday. attached bone marrow report for your refrence. please sir suggeset what i do now.

I have gone through the clinical summary of your elder brother.
The clinical history and various investigations show that there is a possibility of an underlying auto-immune disorder which is responsible for very low platelet counts.
Low platelet count is responsible for petechiae and blue colored spots on body.
The CT head shows a small sub-dural bled also.
The bone marrow report shows that the bone marrow is functioning properly and the platelets are being destroyed in peripheral blood.
Some auto-antibody is in play causing pre-mature destruction of platelets.
Positivity for ANA is suggestive of an auto-immune disorder.
referral to rheumatologist for further investigations will reveal the type of auto-immune disease and thus management.
Blocking of auto-antibodies will help to platelets to survive and thus thrombocytopenia would be cured.

Hi sir, i want to know treatment line is correct or not because 22 days has complete but still no improvement in platelet count, all familly members are worried so please confirm treatment line is correct or not and how many days will take to recover .

Rituximab injection is expected to improve the platelet count

Hello and welcome again,
As per the clinical summary report, your brother has been given steroids to suppress auto-antibodies if any.
There is plan to start Rituximab which is effective against B lymphocytes which produce antibodies.
I suggest you to wait for the response after Rituximab injection.
In addition, investigations to exactly point out the diagnosis by the rheumatologist will also help.
