Question: Hi so my whole life literally ive been struggling with sores. In my mouth and in my genitals I currently am having a horrible outbreak in my genitals and have about 7 ulcers. I used to see a rheumatologist and she was suggesting chrons disease or behcets what can I do I am dying literally this is so painful

Well the multiple oral and genital ulcers do point more towards Bechets than Crohns Disease as we would expect a few more Gastrointestinal symptoms with Crohns. I would like to ask you a few more queries related to your illness:

1. How frequent you get these ulcers? How many episodes a year/Month?

2. Are they very deep seated? Do they heal with the scar?

3. Anyone in your family with similar complaints?

4. Were any of your blood test done with ANA, HLA B 51, ESR, CRP

5. Any other comorbidities for which you are taking medicines?

As for the treatment I understand you are in quite some trouble. So, if there is no possibility of Herpes(recent sexual contact), I would suggest you start the following medications:

1. Tab Colchicine 0.5 mg once daily for 3 days and then increase it to 1 tablet twice daily

2. Tab Predisolone 30 mg once daily for 5 days and then make it 20 mg and then we will keep reducing it.

Hi these sores in my light especially never leave. I do say they have been less lately but before they were like 5-10 at a time and while one was disappearing more are coming in. And for the genital ones those are about once or twice a year but I’ve never had it this bad. I woke up with about 4/5 more. And as the herpes I was tested for that and it was not herpes. And no medications currently. And how can u start those medications? Sometimes they do leave scars

Thanks for your reply. So from Whatever history you have given, it very much looks like inflammatory ulcers due to BEchets as they do leave scars especially in the genital area. Firstly, we need to understand the mechanism behind this and then only we can plan the treatment

1. It is an autoimmune problem, and the ulcers are due to the inflammation where your body is attacking your own self. Scaring ulcers are more of a sign of a little aggressive disease. So you can take it like Rheumatoid arthritis or diabetes, that we can control the disease and not cure it. But lot of times it does happen that when we suppress the inflammation for long, disease goes silent

2. The medications has to be given for long, at least 6 months to 1 year till you have no more episodes of ulcers. Steroids are the quickest to act and Colchicine works wonders in Behcets ulcers. But at times it does happen on tapering steroids, you get a relapse. That s when we consider another form of drug.
I am not sure if you can purchase medicines from online prescription but you can request your GP to issue you the tablets and you will experience relief in 4-5 dAYS

3. Make sure you avoid smoking and less of red meat as they can act as possible triggers

Is there anyway you can send the prescription to the pharmacy or anything? I do not have a dr currently and due to the virus clinics are not accepting new patients

Well, I don't have a problem sending but since I am from XXXXXXX they will not be accepting the orders as they need registration from there. But you can inquire from any nearby pharmacy if they would accept prescription from a doctor practising in XXXXXXX If they would, you can get me their email. I can stamp and mail if it works for them to at least supply u one weeks medicine. Rest I am not sure how it works in your state there as every US state has their own health policy
