Question: Hi there. I have chest pain and arm pain on the left side, built up mucus I think and spasms in my esophagus quite often. the pain sometimes radiates down into my hand and also into my ribs below the left breast. I sometimes get pain in my neck and shoulders too. I went to Emerge in the end of October and theg did all the tests - blood work, chest xray, EKG, heart ultrasound and a VQ scan but my lungs and heart were clear. They said its probably just anxiety but never gave me a true diagnosis. Very frustrating because Im in pain every day and I%E2%80%99m only 23. my ribs and chest are sore to touch as well. Heart rate is normal.

Brief Answer:
most likely stress related, still further details are needed to proceed.

Detailed Answer:
Hello and welcome to Ask A Doctor services.
I have read your query and here is my advise.

* As per my clinical experience, the multiple symptoms of chest pain, spasm
in esophagus and radiating pain to hand, ribs below breast indicate non
specific disorder of stress induced changes most likely.

However, I would like to review in details with following additional information please.

1. Duration of symptoms with progress since beginning.
2. Are you having all symptoms together or various at times ?
3. How do you get relief every day ?
4. In which particular situations , do you feel that they are at peak ?
5. Mood status changes if any ?
6. Do you have sound sleep or disturbed lot ?
7. Any changes in weight, appetite ?

Awaiting for your response.
Have a great day.
Regards.

Symptoms
1. I%E2%80%99ve had chest pain on and off since October - that was the main reason why I went to emerge. I have severe anxiety and panic disorder. In the very beginning of March I got sick with a really sore throat, fever and chills, deep cough, headaches and my anxiety got a lot worse because of the coronavirus stuff. I went to the doctor and they said I had a viral infection (tonsil stones). It lasted about 3 weeks. Once I felt better after the cold, I got this pain in my chest again, pain in my left arm that radiates down to my hand, random pains in my ribs below the left breast and then I keep having these random esophagus spasms and mucus. Sometimes I wake up with pain in my hips, knees and feet if I don%E2%80%99t drink enough water. I also get pain in my neck and shoulders as well.
2. Usually the symptoms happen at different times. The arm pain gets worse after I%E2%80%99ve went for a walk/worked out. Usually the mucus and spasms happen at night or in the morning. Chest pain is also worse at night or when I%E2%80%99m stressed out.
Thank you!

Brief Answer:
follow up.

Detailed Answer:
Hi
Thanks for the follow up and relevant clear details of the incidence.

* Again I will agree with the stress issue manifesting as acid reflux and
general systemic symptoms involving various body parts.

* Almost any test you will get, everything will be turning out normal on
clinical grounds.

My guidelines for best recovery

- First and foremost thing is to accept that this is stress related issue and
not any major medical disorder to panic about.
Stress is common at any age and can be handled very precisely with
particular maneuvers.

- Deep breathing exercises, meditations and YOGA will be front line therapy
of your cure ; which is time taking process but will give you results 100 %
- Drink more liquids and maintain your hydration.
- Consume balance nutritious diet to combat your immunity level.

- Supplement with multi vitamin tablet or capsule will support.
- Regular aerobics at home ( as it is not recommended to attend gym in the
current scenario of COVID 19 virus ).
- Involve in likelihood of activities.
- Try to read holy books, interesting novels of your choice.

- Try to manage sound sleep of minimum 6-7 hours per day.
- No particular medications are of need at current time, except tablet
acetaminophen when the pain is above a score of 5 out of 10.

* In summary, there is more of life style modifications needed, rather than
medical or medication support.

Would like to discuss further as per your need.
Keep in touch for regular progress of your condition.

Believe me, you are going to be perfectly alright, just follow simple measures and wait for big results to turn around.

Wish you great day.
Regards.
