Question: Hi.I am a 45 year old healthy male. I am suffering from tinnitus in one ear for the last 1 and a 1/2 months now. However presently the tinnitus has reduced significantly and is now very feeble. It was a result of one off exposure to high pitched sound at home and stress. I have been under medication since. Can it improve further and dissipate from here?

medication: After 3 days of feeling tinnitus my first doctor gave me following medication for 10 days as he checked my hearing with a tuning fork and concluded Hearing WNL: Cap Otocap, Tab K Trip Forte, Tab Cogniboost and Cap Karvol. Thereafter I went to another doctor as there was no relief. My Audiogram test was done. After intervals of 1 week, the hearing loss at 4khz improved from a dip upto 35db to 25db and then to 20db. During this period my medication was Omega-3 fatty acid250mg, Tinnex 20mg capsule 2wice a day, Omnacortil( 60mgx6days, 30mgx6days, 10mgx6days) and somprez/pantop40mg.

Now I am continuing with Tinnex20mg twice a day with Omega-3 fatty acid250mg once a day prescribed for two weeks.

Thank you for your query.

1. It is good to hear that the Tinnitus has decreased and is feeble now.

2. If your Noise Induced Hearing Loss (NIHL) seen as a 4 kHz dip in the left ear is within the last 1-2 months, you may benefit from TransTympanic Steroids and HBOT (HyperBaric Oxygen Treatment).

3. Ideally Tinnex (caroverine) is given as an injection over an hour or so followed by 80mg per day in two divided doses for 2 months after the injection.

4. Initially, these is spontaneous resolution seen in many patients or up to 50% of the initial hearing loss but further spontaneous resolution without treatment seems unlikely after 2 months.

5. IV steroids can be given in much higher doses (up to 1000 mg daily) compared to 60mg orally and can be considered for 3 days. However that requires indoor admission.

6. TRT (Tinnitus Retraining Therapy) or sound therapy can be considered if there is no more improvement with medications.

Thx for the advice. As the problem is about 1 and a 1/2 months ago I am still under 2 months. You have suggested 3 options 1) trans tympanic steroids and HBOT 2) tinnex injection with 80mg a day dosage 3) IV steroids in higher doses for 3 days with indoor admission Dear doctor kindly suggest which option is best suited and since this treatment is unlikely to be available in XXXXXXX where I am located, please tell where in Delhi/NCR and which hospital/doctor should I see for this further treatment.
