Brief Answer:
good but not perfect

Detailed Answer:
Prometheus IBD panel is good to give guidance to the doctor. Overall it is 81% accurate in diagnosing Crohn's disease.
I saw your report, and the only markers that are positive are the ASCA's (plus some mutations noted on the center column that may predispose you to have the disease).
People with ASCA are more likely to have Crohn's however this is not diagnostic by itself. Your ASCA markers were just slightly elevated, so everything would need to be put into context. If you have symptoms (diarrhea, blood in stool, etc), and endoscopy and or colonoscopy are suggestive of possible Crohn's then it is quite likely you have the disease since you also have positive blood tests. If your endoscopies are normal, then i would take the Prometheus panel results very carefully.
Sometimes patients also have imaging studies abnormalities (CT scans or MRI) and this could also guide with the diagnosis.
In conclusion you can't get a diagnosis of Crohn's based on the blood test alone. Your symptoms and findings on endoscopy/colonoscopy/imaging studies also need to be taken into account.
Hope that helped, let me know if you have further questions
