How Are Febrile Seizure And Recurrent Fever In A Child Treated?

Question: My son is 3yrs old. he got his first febrile seziure at the age of 1yr for 15-16 times for 2-3 secs duration everytime and that he had fever also. after that we admitted him in hospital for fever. doctor said with fever its ok and nothing to worry . till July 2017 he was fine. but in Aug he started with absense and droping seizure and one day become quite serious he became senseless and body stiff etc. we admitted him and done EEG with negative report (attached). MRI was also done and normal. doctor prescribed 3ml * 3 times sodium valporate. recently we visited doctor again to review and done EEG again (attached) . we mentioned every thing but doctor said this time EEG is better than earlier one. improving. suggested to continue with sodium valporate 6ml *2 times.

since his last admit to till date he got 3-4 times seizure with fever. one time droped also. that time we sprayed midazolam. i heard that during continuation of sodium valporate it should not come but it happened. what i observed is that he is getting seizure till date only when fever is about to come or having fever. Fever is like a big enemy for him. we feel very helpless when seizure comes. there must be something when fever comes which doctor unable to understand. his weight is 14.5kg and hyperactive also. please guide me.

I have gone through your question and understand your concern.
Febrile seizures is mostly associated with fever.
The age group and conditions matches febrile seizures.
The cause of fever may be many.
Simple viral fever, upper respiratory tract infection can also cause seizure precipitation.
In child such fever are common.
For prevention of such episodes you should start medications of fever as soon as fever starts.
Cold sponging can also help.
Valproate is ok and dose if needed may be increased too.
Most of the times seizures control on medication but not always.

please check the EEG report sir. it is febrile seizure with complex partial seizures. i want know as he is on sodium valporate then seizure should not come even in fever. whenever fever comes he get sometimes seizure like
febrile or complex type and his heart beat become fast. yes we do the treatment for fever but concern about seizure only. whenever fever comes his head becomes hot first and too much rather than body.

many people said under valporate seizure should not come even in fever and it is not good. some saying use frism tablet when fever comes. under sodium valporate if 2yrs no seizure then its better. but whenever come from that day again 2yr count will start.

please check the EEG report sir.

please tell one more thing that can we give pneumococcal vaccine?

Hello,
EEG is abnormal.
Mutifocality points towards atypical febrile seizures or some other seizure type.
Valproate needs to be continued. Fever leads to precipitation of seizures even during continuation of medications.
Most febrile seizures stop by age 6 year. Continuation of medications in fully controlled seizures may not be required after this age.
Try to prevent fever and treat early.
Vaccination can be done.
