I am Dr kakkar (Dermatologist and Venereologist). I have noted your concern.
For dandruff I suggest you to use a ketoconazole and salicylic acid based shampoo e.g either Phytoral - SP shampoo or Salisia - KT shampoo. Lather it well on to wet scalp and allow it to act for 7-10 minutes before thoroughly rinsing it off.
Hair fall could be a part of the problem of irregular menses.... How long since you have had this problem of irregular menses?
How long since you are having hair fall?
Do you also notice your hair getting thinner and widened partition as compared what it was a few years ago?
Is there a problem with coarse facial hair?
e.g upper lip, chin and side locks area?
Any history of acne?oily skin?seborrhea?

How long since you have had this problem of irregular menses? since my menses started it has been irregular only. There is a gap of 2-3 months always since I was 14 yrs old
How long since you are having hair fall? This happens mostly in winters relatively lesser in summers but it happens. I have a hot water bath. So sometimes I feel is that the reason behind my dandruff & itchy scalp? But you can say the hair fall is always sometimes more sometimes less. Right Now it very heavy hairfall. Shampoo used right now is Loreal total repair 5.

This is cause of continuous hairfall. Richfeel people said my follicles are weak unable to hold hairs so hairs are falling from roots itself.

For dandruff I suggest you to use either of the shampoos I mentioned. They are available on prescription from a chemist. Dandruff /seborrheic dermatitis flares up during winters and yes, hot water bath exacerbates dandruff and itchy scalp. So you must use one of these antidandruff shampoos for keeping your dandruff under check, specially during winters. Later you may use Sebamed's anti hair fall shampoo.
In addition to the shampoo I suggest you to use a serum e.g either Redenser hair serum or Q - sera hair serum. Use it on your scalp. Around 2 ml every night.
I also suggest you to take an oral supplement e.g Revilus tablet or H-Vit forte tablet Or Keraglo XXXXXXX tablet, once a day.
These are all prescription medications that are available with a chemist.
