Question: Hello doctor

History of lscs three years back ,followed by a sinus tract in anterior abdominal wall,there was no discharge ,but was doubted of tb.att was also taken ..my latest mri reports are enclosed ,please guide sir

Hello,

As per your attached reports, You have degenerative spondylosis & disc disease in lumbar spine. It is affecting L4-L5 and L5-S1 levels.

It occurs due to repetitive overload or stress to the disc & it increases the risk of disc herniation & spinal canal stenosis.
For this condition you have to do stretching & strengthening exercises in which you have to stretch tight muscle & strengthen weakened ones which will alleviate stress on facet joint and disc.

- Maintain optimum body weight,if you are obese
- Avoid lifting heavy weights as it imparts more strain on your back
- Change your daily activities so that you avoid movements that can cause further pain, especially bending forward and lifting as it will worsen the disc protrusion. if you want to bend forwards, do it with your hips and knees flexed.
- Use a lumbosacral back support to stabilize your spinal muscles
- Intermittent pelvic traction might be helpful to cause a regression of the protruded disc
- After the initial pain has subsided, remedial exercises to strengthen the spine are very important to restore normal spinal movements and muscle strength. The main aim of exercise is to encourage mobility, reduce muscle spasm, prevent further deterioration in the muscles and speed recovery.
- Use analgesics like advil or aleve and muscle relaxants to relieve pain and muscle spasm, as needed. You need to consult an orthopedician and get these prescribed according to the severity of your symptoms. Vitamin B12 supplements & Pregabalin may be helpful in controlling the neuropathic pain.

Thanks doc..

What about the sacroilitis part..i have consukted an orthopedian and he prescribed be nucoxia d 3 for a week with nuroday k2.before that i have consulted a rhumatologist and she prescribed me saaz 1 mg daily

My last ultrasound report says i hVe panniculitis and altered echogenity of abdoimnal wall i fat in left i lnfraumblical area

Are all these problems intrrrelated sir???is this growing tb or some auto immune disorder

Whim should i xonsult for all this and medicines should i take

Hello,

Sacroiliitis is an inflammation and can be controlled with pain killers and exercise. The medicines you have been prescribed are correct. You can get an additional muscle relaxant prescribed for relief. Do a hot compress and apply local analgesic gel also. Regular exercise under the guidance of a physiotherapist will help in early recovery.

I have seen the additional reports. The ultrasound findings are not worrisome. Altered fat echogenicity can be due to scarring. All these are not related to TB or the back pain. You need to go for a regular follow up to your orthopedician.

The area that has been mentioned in USG is quite bothersome?? What could be the reason for this scarring? (was last reported in my MRI too attaching pic)


I am getting relief in my back pain for last five days after taking nucoxia as prescribed by ortho doctor, but Saaz was prescribed by rheumatologist. Can both be taken together,
For how long this can be taken( both medicines)


My last usg reported panniculitis and i have taken att twice with levoflox additional second time for a long period?? Is sacroiliitis or back problem arised because of this??

I m attaching all prevjous reports too sir. Pls guide me the right path??

Should a biopsy be done if it is either fat necrosis or TB.

Can cancer present picture like this?? Pls suggest differential diagnosis

Hello,

A small area of fat with altered echogenicity can be due to scarring due to a trauma or surgery. It can be a benign lipoma also. This can happen due to herniation also. This is not related to or suggestive of cancer or tb. This usually remains as it is is for years without causing any problem. You can get the area surgically removed and send it for biopsy to confirm this. The other differential diagnosis can be lipoma, scarring, fibroma, etc.

Both Nucoxia and Saaz can be taken together. The duration depends on severity of symptoms and extent of relief you get. Do get yourself clinically examined by an orthopedician after a week to find out if this needs to be continued or not.

ATT does not cause sacroiliitis or a back problem. The back problem is a posture related problem and can be related to low levels of calcium and vitamin D3.
