Posted on Wed, 10 May 2017
Question: Grade 3 isolated pcl tear. Would surgery help keep arthritis at away for longer. My Ortho sent me to a surgeon who did not look at my MRI but decided I didn't need surgery but everything I'm reading says my PCL won't heal on its own

Brief Answer:
Conservative treatment is more appropriate.

Detailed Answer:
Hi,

I have read your query.

I want to clear you the point that, all that depends upon patient's general condition, whether surgery is required or not, or if there are more chance of arthritis in future.

As you say that you have visited orthopedic surgeon, he must have done your physical examination and advise you conservative treatment.

Once thing I want to clear your that surgeon or doctor treat patients along their reports and only on the basis of report or investigations etc.

I don't know why your doctor haven't seen your MRI report as you said.

Surgery, I usually advise my patients only if my patients complains of associated with the feeling of the knee giving way or if pain is reproduced when the posterior cruciate ligament is stressed by attempting to bend the knee.

Other wise I mostly adivsie my patients for conservative treatment like, ice and heat treatment, electrotherapy e.g. TENS and ultrasound, manual therapy and exercises.

I also advise specific rehabilitative exercise program which may include quadriceps and hamstring strengthening, gait re-education and balance training using wobble boards.

One thing more important is knee support or hinged knee braces as it will provide the most support for knee ligament injuries.

So if you follow strict conservative advise as I have explain to you, then you may not get arthritis early and your life will go on normally without surgery.

Atlast, if you have severe pain even after conservative treatment, you can change you surgeon and visit another arthroscopic surgeon for better advise or surgery if needed.

I hope you got the point.

Thanks,
