How Can High TSH Levels And Obesity Be Managed?

Question: Hello doctor , i am XXXXXXX shah. I have hypothyrodism. I go for checkup on 6 months interrwal as per my endos instructions. I dont see any change in me from the day i take the thyroxin to till date. Everytime when i visit my endo, she tells me to cut down weight. I dont see any change at all, but as she is one of the best endo in XXXXXXX so as per my moms choice i continue going. This time she gave me 75mg thyroxin and neurokind d3 on alternate day, which i couldnt continue for long. But i took 75 mg regularly. Now she has suggested me calcium tsh and cholestrol tests. Except tsh i see all are normal. Tsh is 44.2 against the range of 0.5 to 4.4 . I want to know ur opinion. Is it just about my will power to loose weight,? Or some diagnosis needs more correction? Tomorrow i will be meeting my endo after almost 1 year. I feel like not seeing to her as it doesnt change my condition.

The TSH level is high.This level suggest that your hypothyroidism is not stabilised with the actual dosis of thyroxin and this dose must be adjusted.
It is necessary to increase the dosis of thyroxin and repeat the TSH level after 6 weeks.

The overweight or the difficulty to loose weight ,can be a symptom of your hypothyroidism and only when your thyroid function tests will be normalized,you will loose weight sucessfully.So,is not just your will power but is a result of your low metabolism due to hypothyroidism.

Ok madam thanks for the response. Madam my endo suggested 125mg thyroxin. She advised to have insulin resistance and sgpt tests. Most frustrating thing is earlier while i had normal level of tsh, that time also i couldnt see change in the fatigue or other symtoms. All things were same. I want to become fit and healthy. I have a kid of 15 months. I am very enthusiastic person, i dont want to become looser due to such health issues. Please suggest something to become more healthy. Thanks again.

I can't read her writing but I'm sure that your doctor has prescribed Janumet.

Janumet contains a combination of metformin and sitagliptin. Metformin and sitagliptin are oral diabetes medicines that help control blood sugar levels.

Your tests results show severe insulin resistance and this can be attributed to diabetes but even to your obesity.

I didn't see your HbA1C result.This is very important for the diagnosis of diabetes mellitus.

Yeah madam you are right that is janumet. HbA1C, i will be doing after 1 month. She has not recommended that. Meanwhile i have changed my lifestyle and trying to loose my weight. Currently i am taking 3 pills . 1. Janumet once a day 2. Metlong once a day and 3. Neurokind d3 once a week. And she has recommended to have b-12 injections. Thanks a lot for your kind help.
