How Can High Blood Sugar Levels Be Treated Despite Being On Volix?
Question: hi, my father in law is diabetic. for past 5-6 months his fasting sugar level is 140mg/dL. he used to take only glycomet gp 3/850 twice daily. On consulting doctor, he had been prescribed t volix 0.2mg in afternoon with food. but his sugar level is not coming down . please suggest what to do? does his dosage need to be increased? thanks

I can understand your concern. What is his last HbA1C level and postprandial sugar level? If the HbA1C value is much higher then oral antidiabetics are not much useful. Kindly follow up with the detailed history.
I think your physician already increased dose by adding the tab. Volix. I'd suggest you repeat test after 2 months and check fasting and postprandial level. If again it is increased then we can think to increase the dose or change to another class of antidiabetic drug as per Hba1c level and sugar level.
You can increase the dose of the tab. Glycomet gp to 3/1000 twice a day, if he is having typical symptoms like fatigue, polyuria, polydipsia, after diabetologist consultation as per the sugar level. I'd suggest you monitor sugar level at least thrice a day. Along with that do urine creatinine and fundoscopy examination if not done earlier.
Along with that consult with a dietitian and plan his diet as per a proper diabetic chart. Do regular exercise, limit your calorie intake, take a balanced diet. I think this will clarify your answer.

thank you for the detailed reply. last hba1c test was done in Aug 2017, and value was 8.10 . I will get the test done again along with other tests. he is not having polyurea or polydipsia

thank you doctor. he is starting on 3/1000 from today. he complains of frequent urination after taking volix and want to discontinue that. is it advisable?

In your case it may be due to diabetes, But we have to rule out other possibilities. Let me know whether does he has burning micturation, pain in abdomen, blood in urine, fever? Kindly follow up with detailed history.
I'd suggest you to do urine routine, culture sensitivity, ultrasound abdomen and pelvis. Treatment mainly depends upon the underlying condition. Strictly monitor sugar level thrice a day. Plan a proper diet chart. I think this will clarify your concern.
