default
Posted on Wed, 4 Dec 2019
Question: My Nephew is 19 years old. A year ago he complained from persistent headache and was thus diagnosed with hypertension. Kidney and liver functions were good and thus it was justified by stress and was on light concur 5 for a while till he was normal.
This week it revisited and his early morning readings reached 190/135 mmHg and all day long never below 160/110 though first hand advice from the doc was to take the concur 5 twice.
We carried out Echo and I attach hereto the report and are to conduct new kidney and liver function anslysis as well as lipids and also might do XXXXXXX scan on the Aorta and Kidney artries.
Would you pls give me your readings of the case and how serious it is.

I would explain as follows:

Detailed Answer:

Hello,

I passed carefully through your question and reviewed your uploaded reports and would explain that these cardiac changes are related to uncontrolled high blood pressure.

These cardiac ultrasound findings are not serious, but the most important issue is to find the possible cause of high blood pressure and try to manage it, in order to stop the progression of these cardiac changes.

You should know that these blood pressure values are really abnormal at his age and it is important to investigate for possible causes of secondary hypertension.

In this regard I agree with his doctor recommendations on performing kidney and liver function tests coupled with an angio CT scan of the aorta and renal arteries.

In addition, I would recommend also other tests:

- aldosterone and cortisol plasma levels for possible adrenal gland dysfunction
- urinary metanephrines for possible feochromocytoma
- thyroid hormone levels for thyroid dysfunction
- blood electrolytes for possible imbalances.

You should discuss with his doctor on the above tests.

In the meantime, I would recommend starting antihypertensive therapy in order to have a better management of blood pressure values.

Hope you will find this answer helpful!

I remain at your disposal for any further questions whenever you need!

Kind regards,

Dr.Iliri
