When there is blood in the urine, it is usually from somewhere in the urinary tract - kidneys, ureters, bladder, urethra, and can be from a number of different causes, the most common being urinary tract infection or stones.
When blood is in the stool, it is usually from somewhere in the gastrointestinal tract - typically below the level of the stomach, because if its from he stomach the acid makes the blood look black and tarry rather than bright red. However if bleeding is brisk, it can be bright red.
When blood is in both the urine and stool, it can be from some other source and get mixed in to the urine or stool, such as blood from the vagina can look like it is part of the urine.
Nausea can be associated with a kidney infection - usually people feel some flank pain and have a low grade fever if that is what is going on.
Nausea can also happen with gastrointestinal problems.
To sort this out, I recommend you go in to be seen and have an exam. The doctor can check your stool for blood with a guiac stool test during the exam and check for hemorrhoids and vaginal causes. Also need a urinalysis. In the office, the doctor can do a quick dipstick urinalysis to look for blood and signs of infection and then send the urine for a complete urinalysis and culture (the culture would be if there are signs of infection).
