How Can One Overcome The Chances Of Miscarriage After Two Such Instances?

Question: hi my wife had undergone IVF FET on 14 Jun 18 and after 18 days we had gone for Beta HCG test (1092) followed by USG on day 21 in which it was confirmed that she has conceived and after 2 weeks again in the scan it was found out that there is no heartbeat and fetal Pole we were asked again to go for BHCG test. Repeat BHCG was around 16225. The doctor performed another scan and confirmed that it was a blighted ovum. What are the reasons for BLIGHTED OVUM??? How can it be avoided in next FET planned after 3 months. The doctor prescribed MIFEPRISTONE 200mg and misoprostol 800 mg. After taking Mifepristone there was bleeding with lumps falling off. is it necessary to take misoprostol medcine also??? This is my wife's second miscarriage the last one was in her 5th month due to incompetent cervix. please advise how can we overcome this problem of recurrent miscarriages??? please advise.

Firstly, I offer my sincere concern for what you are going through.

The causes of both the pregnancy losses could be different. While cervical incompetence can be handled with observation, rest, encirclage etc., early pregnancy loss needs further evaluation. The biopsy of the products helps to know the cause in the majority. Infections, abnormal genetic patterns, the influence of the environment, drugs etc., anatomical defects and so on, have to be considered.

A blighted ovum is a pregnancy without fetal development. The exact cause is unknown in many cases. Chromosomal defects, consanguinity, genetic errors are some causes.

If your consultant has confirmed the completion of the abortion, there is no need for any further medication. A follow-up sonogram is enough to check for the completion of the process.

As I mentioned above, since you experienced two different conditions, further management depends on the careful evaluation of the situation. However, as your wife is young, there is no need to worry. The chances of recurrence are less and the possibility of a healthy pregnancy is more subsequently.

Thanks for the reply. My question now is whether my wife should take misoprostol 800 mg after taking mifepristone as a combo???.... please advise.

Ideally, the combination has to be taken as prescribed. However, since your wife seems to have expelled the products, I would suggest continuing with the course after your consultant examines her.

If the bleeding has stopped now, it is more likely that the process is complete.
