my father felt dizzy this morning at church, he sweat and passed out for like 10 seconds.
He is taking high blood pressure meds, along with heart and diabetes meds.
he took his morning pills without much eating and he woke up early at 5am to prepare for the ceremony.
please advise if this is orthostatic hypotension specially that we went to the ER and all tests were normal (ECG, blood flow test...)

As your father takes antihypertensive drugs and antidiabetics it is more likely that this situation is related to his current therapy ( low blood pressure or low fasting glucose ), as he didn't eat enough or was dehydrated while working and sweating.

Physical activity and and psychological stress has played an important role in all this clinical scenario too.

So, it is important to monitor his blood pressure values and fasting glucose levels in the next days.

In case of low blood pressure, he may need to lower antihypertensive drugs doses.

In case of low fasting glucose, a decrease in anti-diabetes drugs doses( oral pills or insulin) may be needed.

Anyway, I don't think that this situation will repeat, if he avoids prolonged standing up position and straining physical activity and eats adequately in the next days.
