To diagnose I would suggest some basic tests Like:
1. X Ray Of Back bone
2. MRI Lower back if x rays do not provide any clue
3. Blood tests Like CBC with ESR, R A Factor , ASO CRP Blood Sugar , S Vit D3, S Calcium, S Phosphate ,S Alkaline phosphatase .
In the meantime as empirical treatment Start pain killers , Local Heat therapy ,
