default
Posted on Fri, 23 Jan 2015
Question: I am XXXXXXX from XXXXXXX XXXXXXX
I Had sexual exposure on 15 th august 2014 with condom but I did oral sex without protection by licking her vagina , I don't know her hiv status, except that oral sex every exposure is with condom only
My last sexual encounter was September 6th
I had HIV 1RNA PCR Test qualitative on 21 st. September which came viral load not detected . Detection limit of test is 70 IU/ml .this test is done 35 days post oral sex and which is 15 days from last sex encounter which is with condom .
Again I tested on 11th October 2014 for HIV 1 DNA PCR Test qualitative and P24 antigen and antibody Which is 55 days from oral sex exposure and 35 days from last sexual exposure which is with condom
These test also came as negetive
Hospital people told that they use 4th genrarion kit for p24 antigen and antibody
Further to which I had test on 14 . 11.2014 which is 90 days from first exposure where vaginal sex is with condom but oral without condom 70 days from last exposure which is with condom
Again on next day 15.11.2014
I tested again RNA PCR for HIV 1
and ELFA test which is fourth gen test , this is 70 n 91 days from first n last exposure
All came back negetive
Now on 10.12.2014 which is 115 days from first exposure and 94 days from last exposure I tested again in BGS global hospital using ELFA 4th gen test
Got report next day as negetive
My quiery is after all these test still is there a chance of hiv
As I see on net it shows hiv test post 3 months are conclusive but some other sites say 6months or even I have seen things like window period is 10 years
Please clarify I am emotionally very low n scared now
During my first encounter I had vaginal sex using condom but oral sex where I used my mouth with out condom in her vagina
Except that all other exposure is with condom
In these 90 days period I never had any sex with unknown partner
Please let me know can I resume my normal life based on above test results
Is 90 days window period is correct or it is beyond 90 days
I don't have any symtoms other than burning sensation only during sweat in under arm
I got checked from doctor he told no lymph node in my body

Please help me with detailed answer about my status and window period

Also Does following medication has any effect on hiv window period
I have tested for Hiv ELFA hiv 1 & 2.
4th genrarion ELFA testat 95 days from sexual exposure
Test result says hiv negetive

Tablets which I took during window period are following
Chymorol forte
Citrazene
IBUGESIC PLUS
2each per day took these for 4 days
Doctor gave these tablets for ear pain

Please confirm does these tablets delay or have any effect on accuracy of HIV test I did
from last week I have sore throat n cold
Do I have to repeat hiv test any time or is my test conclusive

Please confirm
My marriage process is going on
Help me

Once a while I get pain under arms
Pain is not continuous
Please confirm on these queries
I am really confused about window period
Please let me know about above quiries in details

Confirm do I have to test for hiv again n again or my tests are conclusive that I donot have hiv

I have tooth decay not bleeding gums from many years so does tooth decay pose any chance for risk

I did check condome after every deed and found no condome were broken
But I am scared wat if pin hole is in it
But as per websites pin hole will expand while wearing leading to rupture in it please confirm

Kindly confrim for all above points also confirm that whether test conducted by me post 90 days is conclusive or do I have to take test at 6 months

Brief Answer:
You are an HIV negative person

Detailed Answer:
Thanks for your question. I am Dr Matin and will be answering your queries.
You have provided a very detailed account of your problems and surely I will try to help you out there.
First thing first : I want to assure that you are an HIV negative person as per your several of tests .
i) These are all 4th Generation tests which pick up 95% positive cases by 1 month of exposure and 99.9% by 90 days of exposure.
ii) These 4th generation tests have decreased the 'window period' from 6 months to 3 months and so universally 3 months window period has been accepted as a 'cut off' .
iii) Naturally you don't need any to go for any further tests. So no worries over there. You can safely go on with your marriage plans.
iv) HIV can't be judged by symptoms as the symptoms of sore throat, ear ache , tooth decay , gum bleeding ,pain under arms including with fever, loose motions ,weight loss, itching, gland enlargement are found in non-HIV persons too, and the only way to confirm HIV is the tests for HIV ( which are all 'negative' in your case even after the specified 'window period') .
v) No other disease process or any medications do have any effect on the positivity or negativity of HIV tests.
vi)Though unprotected oral sex is an extremely low risk activity but you have tested negative already so there is no way you can be 'HIV positive' .

So don't worry , there is no HIV in you . Be happy and go ahead with your marriage plans.
Hope this helps.

Thank you for your assurance

Tooth decay is from long time even before my exposures my question related to this is having tooth decay n doing oral has any chance of getting Hiv at any point I didn't have gum bleeding
Coming to condome part I did all sex with condom which were in tact after deed also
But I am unaware of pin holes in it it's my doubt as people say even small holes expands when it is worn n it can be identified is it true

Please consider above points n confirm me that is my result is acceptable or do I have to recheck at 6 months
Please help me your answer matters a lot
You said at months mark it is 99.99% wat about remaining % in this

Brief Answer:
My answer remain the same

Detailed Answer:
Thanks for your follow up questions.
In Science we go by evidence . What I suggested was the probability /chances and nothing is 100% in medical sciences . Now what to do away with this 0.1%---the remedial process -You have already done . We do multiple tests to increase the sensitivity (power of picking up + cases if they are positive) Your all tests @ 90, 91 and 115 days post exposure have come negative which makes up for that 0.1%.
Once you are a negative person, those pin holes/gum bleeding do not not matter in your case.
I suggest you to see your physician /dental surgeon to treat your gingivitis(gum problem) and tooth decay. I guess these become more important things to address ,once you plan to get married ('bad odor' is a big negatives for spouses ).
You are an HIV negative , I reiterate it and you should look forward to much brighter aspect of life.

Hope this helps.
Regards

90 days 91 days & 115 days are for oral exposure
But for my last exposure it is 70 71 n 94 days
My last exposure was with condom it self
Which was in tact same case in other exposures before that
So should I test for this case again or one test post 90 days is enough for them to be conclusive

Any day do I have to re test for hiv in future for these cases or no need for further testing

Brief Answer:
Answer remains the same

Detailed Answer:
Thanks for your response .
Let us understand first that there are no separate HIV tests for different mode of transmission -oral or vaginal , with or without condom. Be it any mode of transmission , the fact of the matter is that virus goes inside your blood and produces antibodies which we test for. We now test the antigen as well for almost 100% confirmation (which we call 4th Generation test)
Saying the things like '' '90 days 91 days & 115 days were for oral exposure
and for your last exposures -- 70 71 n 94 days'' have no scientific basis .
Pl forget about these number of days .
The important thing over here is that you tested negative four times on 90, 91 94 and 115 tests post exposure beyond the specified window period of 90 days and this proves you are an HIV negative person.
Once you are a negative person , there is no need for further tests .
I hope, I answered very clearly about your queries and there are no further doubts.
Pl go ahead with your life and enjoy it.
I close the thread over here .
