Question: how do I distinguish between sciatica versus a possible hamstring strain extending from just above knee to high calf area, if hamstring why is taking more than weeks to reduce pain complaints after physical therapy. also have noticed discomfort in buttocks after streching not burning or tingling but more pain

Hi,

Sciatica can be an important cause of muscle spasm. Hamstring strani IS muscle spasm. So, the two are not exclusive.

I would insist you to get a X Ray of Lumbosacral spine (AP and lateral) done. That will help in ruling out sciatica.

Hot compress should provide relief. The vicious cycle of pain and spasm needs to be broken and I would insist you to take paracetamol or acetaminophen 500 mg two tablets together twice or thrice daily for next 3 days. Physical therapy can be helpful but can take quite some time since it needs to strengthen the muscles first.
