Your sperm count is low. It is 12 millions per ml. Normal is 20 millions per ml.
Your sperm motility is also low. It is only 10% at half hour and 5% at 2 hour. In normal report at least 50% sperms should be motile after 1 hour.
Spermiogram shows that 60% sperms are abnormal(means 40% are normal). In normal report at least 30% sperms should be normal.It is ok. However overall sperm count is quite low.
Fertomid (clomiphene) is the drug which can increase sperm counts as well as mortality by stimulation of FSH and LH hormones. Though clinical trials have not been done for this, most of the gynecologist suggest it is effective in many patients.Sometime it will take 6 months to show optimum results. You can continue the same under your doctor's guidance.
Ubi Q 300 is an enzyme-amino acid preparation.Id contains LArginine 100 mg Selenium 100 mcg Tocopheryl Acetate 100 mg Ubidecarenone 100 mg. It helps in decrease in ubiquitination process during sperm production in epidydimis. (only assumption on theoretical basis, no confirmed data available) Increased sperm ubiquitination is INVERSELY associated with good semen quality parameters. However no confirmed clinical trial has shown its effectiveness. In my opinion no need to take it further.

Thank you. Appreciate it. I find a bit of negligiance on the part of the physician who got it tested. Usually the report gets generated in XXXXXXX 7 odd hours since the time one gives a sample. But in this case, even after asking the lab for the report after 12 hours since I gave the sample, they didnt not have the answer moreso could not even tell me orally on the readings. Report was generated after 2 days as you can see on top of the earlier attached pdf (report).
Question 1 - Does the sperm sample stay alive and in ideal condition for testing after this delay and is it a negligence on the part of the lab ?
Back in October when I had done a sperm count test. the readings were slightly better - 18 millions and Total sperms per ejaculate was 54 million and 50% spermatozoa alive post 1/2 hour and Active sperms in the ejaculate (at the end of 1/2 hours) was 21.6 million.
Question 2 - So I want to know whether I should go ahead with the same tablets as prescribed to me or change to any other ?

But I think report is genuine probably though printed report given late.
It was collected at 10.35 am and examined at 11.05 am. So it was examined immediately.And if by chance it was reexamined, semen remain viable in storage at 15-19°C (59-66°F) temperature.So not likely negligence.
However still if you feel doubt , you can go for testing in other reliable laboratory.
Ans 2.
Medicines are ok at present. Specifically go with clomiphene. Considering worsen semen report than in October I feel you should go for testosterone,FSH and LH level and then consult gynecologist/surgeon.(yes gynecologist - they are more accustomed with these hormones problem and with use of fertility medicines).
