How Important Is It To Go On Medication When BP Is 130-140?

Question: This is my question. I take my blood pressure at home and a lot of times I got a reading over 140. This reading is usually middle of the day if I take the blood pressure in the morning and in the evening I get readings in the 130s. Somebody once mentioned I should go on blood pressure medication but whenever I go in the medicine I don't feel like myself. My question is how important is it for me to go on medication I am a 36 year old male. Is it possible I can put this off a few years is it something at a level that is super important for me to do this

I understand your concern and would like to explain that you are experiencing mild degree hypertension. Not rarely at your age a certain organ dysfunction may be the cause, like thyroid gland dysfunction, decreased renal function, anemia, a systemic inflammatory disorder, even anxiety may the the underlying issue.

That's why a systematic review of the above mentioned issues is recommended.

Once the underlying cause is detected and treated, hypertension will be resolved.

In addition, the presence of other cardiovascular risk factors would be a prime for initiating antihypertensive therapy (ACEI, ARB, or a calcium channel blocker woul be appropriate at your age).

Furthermore, modifying lifestyle, treating obesity, smoking cessation, avoiding caffeinated drinks may be sufficient for decreasing you blood pressure values.

You need to discuss with your attending physician on the above mentioned issues.

I would wxplain rhat your blood pressure values are considered mildly elevated; wheather treatment is required or just lifestyle and diet is sufficient would depend on the presence of othe cardiovascular risk factors as I mentioned above.

The same strategy holds true also when calculating the future cardiovascular risk. If additional risk factors are present (diabetes, dyslipidemia, smoking, erc.) then the risk renains high and prompt aggressive interventions are required. In such case an adequate antihypertensive therapy should be started.
