Question: Greetings -

A few weeks ago, I received a lapdance/handjob by a stripper (likely also a sex worker), who decided to lubricate my penis with her saliva (what if there was a little blood in the saliva?). She spat twice on her hand and rubbed my penis. I did not ejaculate as I got scared and asked the dancer to stop. My penis was exposed to open air to 1-2 minutes XXXXXXX I did not get a chance to wash my penis with soap and water until two hours later. This took place in an area with prevalence for such diseases.

Two days after this incident I received my first dose of the Hepatitis B vaccine.

I have been under tremendous stress with concerns of getting Hepatitis B from this episode.

I would appreciate your kind thoughts.

Dear Doctor,

I am uncircumcised and there were no visable sores or cuts on around my penis. It wasn't a complete handjob as I had her stop. If I can say, it was a brief and gentle rub. The only thing is that I was unable to wash my penal area until two hours later.

I can make out the level of stress you have after the episode. Therefore after analyzing the complete situation along with other facts let me first and foremost reassure you that there are no chances of Hepatitis B in your case.

Let me give certain important medical facts for my firm declaration:

- In your case the source was by saliva of the lap dancer. One thing is that it is not confirmed that the lap dancer was positive for Hepatitis B or not. SO the status is unknown to us. On one side she might be negative which makes us totally free of all the anxieties. In case even if we conciser her positive then also saliva is not the source for Hepatitis B infection.

- Now even in worst of circumstances if we consider that the lady had some blood in saliva (which is actually rarest of rare possibility) then also the chances of infection are not present. It is because of the fact that you had brief and gentle exposure with no cuts and no sores on penis. So the transmission site is totally intact and the virus even if present then can not penetrate the skin.

- Now in the rarest condition even if the viruses penetrated the skin then also the chances are not possible. It is because of the medical facts that saliva contains certain enzymes which makes most of the viruses avirulent and non infective.

Above and all it is your wisest thing that you got vaccinated for Hepatitis B. I must thank your treating doctor who prophylactically given the vaccine. This makes every possibility null and void. Do let me know if you are previously any time in life vaccinated or not for hepatitis B before.

So just relax and enjoy the weekend. I have provided you all the valid reasons for your negative status as per my experience. Also in my clinical experience I have never ever seen any positive case by your kind of exposure.

Thank you much for your reassurance. It is very meaningful and logical. Would the first dose of Hepitits B vaccine my doctor gave me two days after exposure reduce my risk of the current situation? There are two other doses remaining. I am not a doctor so don't understand the science behind it but would certainly appreciate you sharing your wisdom.

DNA PCR if you want to confirm negative status.

As per the time schedule it is exactly 2 weeks sine the incidence. So the best test at this time is DNA PCR to detect the DNA of hepatitis B viruses.

At 14 days till minimum of 30 days there is no other test which can be most confirmatory other than DNA PCR. So you can go for this test.

As I am saying since the beginning of our discussion that in my opinion it would be definitely negative.
