Regarding INR targets, they differ depending on the underlying diagnosis.
With only heart failure diagnosis (a dilated cardiomyopathy), a level of around 2.5 could be sufficient (above 2-2.1 we have a satisfactory anticoagulant effect).
But, if there exist other additional pathological conditions (such as a very dilated heart atrial chamber, or you have actually an implanted mitral valve mechanical prosthesis, heart mural thrombus, atrial fibrillation, etc.), then the INR target should be higher (above 3 up to 3.5).
In such latter case, I recommend continuing Tinzaparin until we have an INR result of 2.5-3.5.
You need to continue Tinzaparin at least 2-3 days and check again INR value (the minimum time needed for Warfarin to achieve a steady change). Nevertheless, the effect is somewhat individual, that's why you need to check the lab test.
