Brief Answer:
You are a diabetic patient.

Detailed Answer:

Hello,

Once your fasting blood sugar is more then 126mg/dl and PP is more then 200mg/dl you are a diabetic patient and you need to take oral hypoglycemic agents like Metformin etc regularly to keep diabetes in control.
So basically you are a Diabetic and you have to start taking Metformin slow release 500mg tablet once daily 10 minutes before break fast.
Secondly i would suggest you to go for HBA1C test to find out the range of blood sugar during the period of last 3 months.
Diet should consist of 50-60% low glycemic index complex carbohydrates(divided in 4 meals) and 30% protein and rest should be fat.

Hope I was helpful.
Thanks
