no pain urinating or blood in urine the urologist examined me said said i had a upper pole edididymal cyst he did prostate exam and said felt fine didnt have any pain in my testicles
but couple days later finding i have moderate pain in one my testicles hes has me taking 800mg ibuprofen 3 times a day

As per the history you have provided this looks to be urinary tract infection that eventually lead to Epididymo-orchitis and giving you the symptoms of pains as you have mentioned.
I would advise you the following:
Send the sample of urine for routine, microscopy, culture and sensitivity tests- this may give results as you have not yet been on any antibiotic.
Tests of blood for CBC, Sugar, Kidney functions, PSA and advised.
Ultrasonography of abdomen and scrotum along with Color doppler examination.
Antibiotic cover may be needed as per the reports - needs prescription.
Take Ibuprofen only when in pain.
Take plenty of oral fluids.
Give scrotal support.
Please get these done to get an appropriate diagnosis so that a proper management can be given.
