Brief Answer:
A number of causes

Detailed Answer:
Dear Ma'am, I have gone through your question and I understand your concerns but this could be due to a number of reasons including gastritis, pancreatitis, pericarditis and pleurisy..If you frequently use pain killers and the pain is intensified on empty stomach then this suggests gastritis.. For which I would recommend a stool analysis to detect h.pylori infection; the most common cause of gastritis.. If gastritis is confirmed then it should be relieved by omeprazol once daily and ranitidine before every meal. If the pain intensifies with eating then this suggests pancreatitis which can be diagnosed by a blood test for the level of pancreatic enzymes.. If the pain intensifies with drop breathe then this suggests pleurisy which can be treated with anti-inflammatory medications or corticosteroids.. if pain intensifies while lying down then this suggests pericarditis which can be diagnosed by ECG.. In the mean while you should take dysflatyl three times daily to relieve the gases which could be the sole cause of your pain.. I hope you find this helpful..

Brief Answer:
Stool analysis

Detailed Answer:
Dear Ma'am, hello again.. I would rather suggest a ct scan for better visualization of your abdominal organs.. Since you are suffering from abdominal distension and gases then the cause could be a flare of ibs or gastritis so I would also suggest that you start with a stool analysis including h.pylori test before preceding with further investigations..I hope you find this helpful..
