Question: Hi Dr
My 7 year old son had an abdominal pain 3 days ago in the middle of the night . I woke up and made him Camomile tea with mint and then he released gases and he told me that he feels better. I have been noticing that he has soft stools for almost 5 days . The first day he had to go the washroom frequently to poop. But then he went less frequently.
2 days ago he had a fever that stayed for one day and now he is afebrile.
We went to his family dr he said he probably had gases but he wants to make urinalysis to exclude urinary tract infection. We made the urinalysis everything is normal except that he has ketones +1 and no glucose in urine . Also the results shows that he has small amount of blood in urine (cannot be seen by naked eye ) , however the RBCs count in urine is 0-2

My questions are how there are ketones in urine without glucose and how there is small amount of blood and glucose he RBCs count in urine is normal?

I want to add the my son has grade 1 autism and he is always hyperactive . Thank you.

I want to add that my son has also mentioned that he felt pain on his left buttock. I am actually suspecting a UTI and the family dr had contacted me today and he prescribed Amoxiclav for 5 days.

Thanks for query .Your 7 years old son has loose motions for 4-5 days and pain in abdomen has been detected to have Ketoine bodies in urine .

Ketone bodies are produced by liver by disintegrating Fatty Acids whenever there is less glucose available in blood as a source of energy .These ketone bodies are utiulized as a source of energy .The excess of ketones are extreted in urine .

The loose motions with pain in abdomen associated with fever is due to Gastroenteritis .Taking Ofloxacin and Ornidazole twice daily along with antispasmodics like Dotoverine will help to resolve the issue .

Presence of 1-2 RBC in urine has no significance and is to be considered as a routine finding.
Since no leukocytes or pus cells detected in urine ,it is unlikely him to have UTI .

Ensure he drinks more water to keep him well hydrated .

It will be better to repeat Urine Examination to get clear your doubt.
