Brief Answer:
Diet and exercise....

Detailed Answer:
I would suggest the following:

1. Avoid all high calorie food. Replace it with fibre rich fruits and vegetables.
2. Prefer to take whole fruit instead of fruit juices and beverages/soft drinks
3. Make sure you keep the salt content low (just enough to taste)
4. Be active; don't sit around.
5. Start quality exercise - brisk walking, running, cycling, swimming, treadmill. 30 minutes a day for 5 days a week is the minimum requirement.
6. Make sure you have ideal body mass index value. If not contact a nutritionist.

These are the minimal activities I recommend my patients who wish to be fit and reduce changes of diabetes/hypertension.

Hope this helps. Let me know if you need clarifications.

Regards
