Question: hello, my wife age 56 weight 107 kg is suffering from diabetes, her fasting sugar is 123 and PP is 215 she is taking Januvia -100 OD with breakfast. she cant take drugs with Metformin because of diarrhea, after starting januvia her diarrhea stopped,now what to do to control her sugar please guide.

Brief Answer: Voglibose. Detailed Answer: Dear Friend. Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern. Her Fasting Blood Sugar is perfectly all right. Only concern is her Post Prandial Blood sugar. Metformin causes abdominal discomfort and diarrhea only for few days. So, treatment options are: 1. To Start back on Metformin, as it is the 1st line drug. 2. Alpha glucosidase inhibitors like Voglibose, that can be taken 2-3 times a day with meals. If she is not comfortable with metformin, she can take Voglibose, it has good response in post prandial hyperglycemia.. Also i advise you to get her diabetic workup done: 1. Kidney Function test 2. Urine for albumin 3. Fundus Examination 4. Lipid profile 5. ECG 6. HBA1c This is my personal opinion based on details available here. If you still have any query you may please ask me. Stay Healthy. Hi, thanks for answer but by mistake i gave wrong fasting sugar reading it is 167 instead of 123 and PP is 215 . Pl suggest treatment. Can she use sugar free tablets in tea? Pl reply.

Brief Answer: Detail follows. Detailed Answer: Dear Friend. If her FBS is at this level, i still feel, that Voglibose would help her. Regarding taking Sugarfree, we advise that if her Kidney functions are all right, and only if she can not take tea/milk without sugar. Hi, thanks. Voglibose comes in .2 and .3 which of these should be given to her ? And how many times in a day? And i ll surely consult a dietician. Would u please suggest a brand name for voglibose. Thanking u again

Brief Answer: Detail Follows Detailed Answer: Voglibose is available with brand names, like Voglikind, Voglistar. I usually prefer to start it in dose of 0.2 mg twice a day with meals, later i modify dose according to blood sugars. If you don't have any further query, please close the discussion and rate it!
