How To Cure Soft Tissue Injury?

Question: My wife had a horse drag a fence into her shin. We got films done and no fractures. A massive he morons on the outside of her shin. Bruising that has pooled down to her feet. It’s been 7 days of elevation, heat etc. The usual waiting game. The pain level is intense still. I cannot touch injury site. However, the pain has traveled down her leg all the way to her feet. An 8 out of 10 she says burning pain from the mid shin area where the lump is to her ankles and feet. Quite worried about her. Obviously we have no health insurance, we are self employed Americans. But my wife is very concerned about loss of use, tissue death etc as she runs track competitively. She is 29 years old. So what is my next step?

Hi I am Dr Gopal Goel , Orthopedic Surgeon.
I have gone thru the details of the Injury to your wife's shin. Shin Injuries are very painful as such.
This is a soft tissue injury causing bruising and hematoma which has travelled to ankle.
As for the treatment you are on the right direction for one week.
I suggest that you add pain killers , Crepe Bandage and gradually start walking on it. Add a combination of pregabalin and nortriptyline ( 75 mg + 10 mg ) daily to reduce burning sensation under doctor's guidance.
Also please take few pictures of affected leg at different angles so I can see and assess.
