Brief Answer:
Tab.pantop 40mg once daily before food is advised.

Detailed Answer:
Hello sir,
All your symptoms are due to hyperacidity and peptic ulcer disease. Once the acidity subsides, your abdominal pain decreases, your appetite improves and weight increases. Take tab.pantop 40mg once daily before food for 14days. If your acidity is not controlled, you can use tab.rabeprazole 20mg daily once before food in the place of pantop.
If your problem persists, you may need to consult a gastroenterologist.
Hope I have answered your question. Please leg me know if you need further assistance. Thank you. Take care.
Thanks.
