Hello!

Welcome and thank you for asking on HCM!

I understand your concern and would explain that your are on anticoagulation therapy (eliquis). This drug is helpful in preventing possible stroke related to different causes.

It is true that in a great number of cases the real cause of the stroke is not found even after performing a lot of investigations.

I would only recommend performing a transesophageal cardiac ultrasound (if you have not performed it yet)to examine better your heart function and structure and to exclude possible cardiac causes (valvular problems of defects), which could be related to the stroke. If such cause is found, you may need to switch to warfarin.

But, if you have already performed this test and no such disorder has been revealed, I would recommend continuing taking eliquis.

A healthy lifestyle and a better control of cardio-vascular risk factors would be also useful in preventing another possible stroke:

- a better control of blood pressure, if you suffer from hypertension
- a better control of dyslipidemia. I recommend performing periodically your blood lipid profile
- avoid smoking contacts
- a healthy diet (Mediterranean diet would be recommended)
- perform a lot of physical activity (walking, aerobics, yoga, etc.)

Hope you will find this answer helpful!

Kind regards,

Dr. Iliri

Detailed Answer:
Hello again!

Thank you for the additional information.

Based on your performed tests, I would explain that your case is a little complicated. It seems that even with all your performed tests, no possible cause have been found (so it is a cryptogenic stroke).

You should know that in up to 30 % of the patients, no possible cause of the stroke is found. It seems that you are part of this group.

For this reason, I would recommend you to continue takine eliquis, even though no specific cause underlying your repeated stroke has been found.

I agree with your doctor decision on starting this drug in your condition.

So, I would recommend continue taking it. I would not recommend performing any additional tests , as it seems that you have been tested for all of them.

Hope you will find this answer helpful!

Best wishes,

Dr. Iliri
