Brief Answer:
Few options

Detailed Answer:
Hi ,
There is possibility of Reoperatiom and doing another sleeve gastrectomy, but it depends on size of first gastrectomy. It is also possible that stomach dilated over time. In such cases, additional sleeve resection may be an option but to evaluate this you should do barium meal test. Second, another surgery can be done, called roux en y gastric bypass which can be done laparoscopically as well. Third option is try to have restrictive diet and see if there in need for surgery at all. Wish you good health. Feel free to ask questions. Regards

Brief Answer:
Hi

Detailed Answer:
Yes, there is obviously remnant stomach dilation or stretching which is not unusual after sleeve gastrectomy. It means that stomach increased in length, although, its width may remain same from surgery. Your weight loss was great, but if it returned to presurgical weight, there is obviously problem with stomach remnant which increased its volume. Now, it is difficult to expect again such weight loss based on diet only. So revision sleeve gastrectomy or gastric bypass or gastric band may be considered.
