There is no set protocol for reducing beta blockers especially if the patient is either having bradycardia or hypotension. I stop the beta blocker immediately and then monitor blood pressure every 2 hours and re adjust or bring in another anti hypertensive for blood pressure control.
Please check the hydration status of the patient as elderly patients often have postural hypotension when dehydrated.
