Question: I am 24 years old (male) height 5feet 8inch , weight 69kg. I am so lean since my childhood but when i turned 21, I observed that i am getting fat on my chest , bums, stomach although my physic is so lean. My nipples bulge out sometimes. I recently met a doctor and was prescribed to run the blood tests and an mri scan on my chest. The doctor said that my testosterone levels are fine but had gynecomastia (small breast tissue) and suggested to meet the plastic surgeon. Is bum fat too a side cause of gyno? Is this a homes issues? Please help out on whom should i be consulting for this? what tests should i be diagnosed for?

Other problems i had are:
daydreaming, not able to focus for more than 2-3 mins, I look very small compared to people of my age and small compared to 8 years younger brother of mine, feel tired very soon if i do some physical work

Hello and thank you for asking!
I have gone carefully through your concern and attached visit report.
Except of low testosterone level i would suggest to check thyroid function and prolactine level. A high estrogen would also be excepted.
An ultrasound of the breast is also recommended to confirm the presence of the breast / addipose tissue.
If all the hormones are within the normal ranges than you should be referred to the surgeon if gynecomastia is an esthetic problem for you.
You should see en endocrinologist at first for the above mentioned hormones.
The other symptoms are not specific and not related to it.

I had thyroid test 5 months back and it was negative, not sure about estrogen.

1) Is there any chance that this could be growth hormone deficiency or pituitary tumor?
2) does bum fat too a cause of gyno, because even i do the gym the fat doesn't go off
2) If not could you please suggest what are the ideal medications or surgeries to gyno?

Yes, it could be due to growth hormone deficiency.
Usually we suggest to exclude the most common pathologies than ( depending on clinical examination and history) other pathologies are checked.
You should have gained weight significantly lately to explain the gynecomastia of obesity.
Regarding to ideal surgery, the esthetic surgeons usually do breast reduction by reducing the excessive fat.It is a safe and successful procedure.
