Question: I am 30 years old , height : 173 cm , weight : 106 kg , i drink , i did an ultrasound of abdomen last week , this is what the report said , Liver Size is 16.5cm. normal in size and shows increased echotexture. No focal Lesions. No IHBR dilatation. In Ultrasound Scanning of Whole Abdomen the impression is - Grade II fatty liver. SGPT ( ALT) is 177.9 . i am extremely scared , how worried should i be ? is this curable ? will i be bedridden ? how long before it progresses to the next stage ? please reply

I have read your query and here is my advise.

* The liver enlargement in size with increased echo texture and SGPT of 177.9 indicate
excess fat deposition in the liver with minor secondary inflammation causing liver
enzymes to alter in amount.

* The progression depends upon multiple factors as life style changes, diet factors,
smoking, alcohol, exercise, body weight, cholesterol levels etc.
But there is no immediate threat of bed ridden condition for sure.

* These changes are reversible with certain modifications in diet and life style, but can
progress if one avoid precautions in future.

My guidelines for your particular concern are

* Diet recommendations :

- Manage to have less amount of carbohydrates in your diet which usually build up as
excess fat inside the liver.
- Cut down intake of spices, non veg, junk foods, excess dairy fat to control body
cholesterol over all.
- Consume more of fibers, green leafy veg, cereals, pulses, legumes, organic fresh fruits.
- Drink more liquids and maintain hydration.
- Avoid aerated drinks and commercial beverages.
- Avoid over eating and prolong fasting episodes.
- Avoid late night dinner and parties.
- Keep your stomach 20 % empty from your satiety value.

* Life style modifications :

- Do regular exercises, walking, YOGA - most essential parts to keep the liver and
digestive tract at its best pace levels.
- Avoid smoking, alcohol if consuming.
- Mange sound regular sleep of 7-8 hours.
- Avoid stress and anxiety.
- Control your body weight, blood sugar and cholesterol ( if cholesterol blood report is
not done kindly get it done and if done please share in follow up session ).
Need of anti cholesterol drugs require evaluation of metabolic panel with lipid profile
report only.

* Follow up strategies :

- In my clinic, I do prefer a 3 month repeat SGPT after dietary and life style changes
and ultrasound at a period of 6 months.

HI doctor , thanks for your reply
i have quit drinking for good and i am walking moderately for 1 hour per day. i have cut my carbohydrate intake too .
i already went to a gastro specialist , he told me to reduce my weight . he also prescribed me vitamin E tablet , sentinel (E-400 IU) , he told me to take 2 doses per day , he said its to prevent fat from causing further inflammation . what is your opinion on this medication ?
