I Am 47 Years Old And Have Been Having Problems

Question: I am 47 years old and have been having problems with PVCs since July (~1,600). I have seen a cardiologist and an electrophysiologist. I have had a stress test, echo, ekg, I was told that I have benign PVCs. I do suffer and have suffered from high blood pressure since I was 18. It is now under control with medicines and diet/weight loss. Two days ago I had a chest xray and the results came back that I have a possible small right lung base thyroid nodule or vascular marking. the heart size is normal. Pulmonary vascularity is within normal limits. There is no focal lung infiltrate nor pleural effusions, There is either a small sub centimeter right lung base thyroid nodule or a vascular marking. I am very concerned as to what vascular marking means? Does this mean pulmonary hypertensions? or lung cancer??? Dr notes it would be prudent to obtain a chest CT scan for further evaluation of lung anatomy. I am scared to death. These PVCs have been bothersome...my bloodwork all came back good--so I don't want to freak out, but would like some information.

Brief Answer:
It sounds completely benign

Detailed Answer:
Hi XXXXXXX

It sounds completely benign. Lung cancer in your age with no previous smoking is extremely rare. I agree that a chest CT with intravenous contrast would provide a final diagnosis. It is most likely ectopic thyroid tissue or a benign tumor.

Hope this helps.

The fact that this is being noted at the base of the lung confuses me as to why it might possibly be a thyroid nodule? I do have three other thyroid nodules in my neck that were just biopsied yesterday. Can you tell me what vascular marking is? I am concerned that is heart related ...thinks like pulmonary hypertension or other more serious. Sorry to be so anxious, but it has been a year different illnesses within the family. I also had six other xrays since February of 2018 and not one of them showed this small sub centimeter right lung base thing. Please respond. I don't want to spend the weekend upset or wondering about this. I do know the CT scan will provide the final diagnosis. Anything your willing to share would be greatly appreciated.

PS. When I was in my early 20s I smoked a few times. I don't think for too long or too much but I wanted to mention.

Brief Answer:
The radiologist may have made a typo

Detailed Answer:
Hi again XXXXXXX

Right, so I think the radiologist made an initial typo (difficult to know without the x-ray). From what I understand, there is a small nodulus, which may be a vascular anomaly (completely harmless) or perhaps something else, e.g. a lymph node at the lung base. The smoking history you mention is without any clinical significance - and good on you! It may also be a hamartoma, but a CT scan will help clarify a diagnosis. I am convinced that it is benign.

Hope this helps.
