I Am 57 Years Old Male. I Had The BPH

Question: I am 57 years old male. I had the BPH surgery done on 9 Oct 2019. Urine flow test is being done every month. Last result shows Umax as 14.8 and Uaverage as 9.8 post surgery. Urologist has advised me Urodynamic test after 6 months. Its invasive and painful test. So my question is : After BPH surgery, is it required to go for Urodynamic test after 6 months ? If so, why ? Is it must ?

Based on the facts that you have posted you seem to have undergone endoscopic surgery for resection of enlarged prostate in Oct 2019 and your successive Uroflowmetry study have revealed satisfactory urine flow .

Normally Urodynamic study is advised in a situation when patient has difficulty in passing urine (Obstructive voiding symptoms ) which commonly occurs due to iatrogenic stricture of urethra after endoscopic procedure .

Since you have normal urine flow and Q XXXXXXX being detected on Uroflowmetry there is no need to do Urodynamic study.

I have uploaded the last Urineflow test report done. Kindly review. Should I continue to go for Urine flow test for few more months ? What should be the frequency of Urine flow test in the future life to come eg once in 6 months or a year ? Is there a possibility that Urine flow may become less again after some years ? Any other suggestions or advise please. Thank you.

I personally don’t advise Uroflowmetry unless patient has obstructive voiding symptoms like difficulty in passing urine, poor urine flow, straining to pass urine etc.

This is the view of most of the Urologists.

Thank you. I have another doubt regarding sexual activity after BPH surgery. Should I restrict that , how long, any precautions I need to take, can it be harmful? Kindly advise.

Thanks for follow up .

Since three months have passed after surgery there is no harm in resuming your sexual activities.However you have to keep following things in mind
1) You may not get erection as hard as you used to get it before surgery .This is common after prostate surgery .

2) You will get pleasurable sensation of climax of sexual satisfaction (Orgasm ) but there will be no ejaculation of semen from penis ,Your partner also may not feel semen being ejaculated in vagina .

In prostate surgery bladder neck is resected making it wide open this results in backward flow of semen in to bladder rather than into urethra what is called as Retrograde Ejaculation ,This is known after prostate surgery and happens with every one who has undergone prostate surgery.
