I Am 58 And Have IGA Nephropathy For The Past

I had an ultrasound in XXXXXXX 2018 which showed normal kidneys. In addition I have a benign prostrate (Grade 2) - no medication. BP is around 130/80. I exercise daily for about 45 minutes.
My recent test results in April 2019 are as follows:
1. Routine urine examination - Urine Protein (Albumin) - Trace , Erythrocytes -Nil (OCT18 - both NIL).
2.RBCs (microscopic examination) - 2.8/hpf (ref range 0-2.4 /hpf).
3. Fasting Glucose 79 mg/dl (Oct 18 - 84 mg/dl). HbA1C - 5.7%
4. Sodium (Electrolytes) - 146 mmol/L (range 136 -145 mmol/L).
5. Electrolytes Potassium, Bicarbonate, Chlorides - all within reference range
6. Total cholestrol - 142mg/dl.
7. Serum Creatinine 1.24 mg/dl (lab range 0.70-1.20 mg/dl). This was 1.30 mg/dl in OCT 2018 and before this was 1.2 mg/dl for the previous 2 years. eGFR works out to 63.
8. BUN (blood urea nitrogen) -12.2 mg/dl
9. PSA - 1.16 ng/ml
10. Micro Albumin in urine - 200.8 mg/L with Creatinine Urine 80.12 mg/dl and ACR at 250.62.
My queries are as follows:
1. Please provide your views on the above test results given my Iga condition.
2. Does micro albumin in the urine fluctuate a lot? I have noticed wide fluctuations in my tests over the years - the urine microalbumin in the Sept 2015 test was 459 mg/dl but then reduced in subsequent tests.
3. Does microalbumin in urine get affected by protein in the diet? I usually eat one egg for breakfast thrice a week, and my diet also includes chicken and fish. Does exercise also affect the level of microalbumin -I usually exercise 45 minutes -5-6 days a week.
Thanks.
your reports are fine
Detailed Answer:
Hi, Dani,
sorry for the delay,
Your test reports are just fine.
i think your disease is stable.
2. micro albumin in the urine can fluctuate. Therefore wee need a trend with repeated estimation to comment on disease progress. if three continuous reports are saying that there is worsening then only any action is required.
3. microalbumin in urine can get affected by protein in the diet. if your diet contains lot of meat products then the urine protein can rise if test is done in next 24 hours. so better is to avoid meat/ nonveg food for 2 days before testing.
vigorous exercise and even jogging can increase the protein in urine. but the effect is transient and is not harmful
best wishes
