Question: I am 5.8 high and I weigh 74 kg of age 24. I always been slim and skinny all my childhood, 3 years back I was diagnosed of Gynecomastia and since then i had developed fat over my abdomen area, thighs, butt and chest but other parts of my body still remain skinny though. I don't drink alcohol nor smoke nor been on any drugs all my life. I look under grown and small compared to people of my age. Recently when I got an MRI of my abdomen, it came out that I had 17 cm of fatty liver. I turned from underweight to overweight in the last two years.
The major problem which I am facing is daydreaming and not able to concentrate. I just imagine some imaginary situations in my mind and gets diverted of work or daily activities I do. Physically I am doing somethings but mentally i am thinking of something else. I could hardly concentrate for 2 mins. I tried going to gym do some kind of physical exercise but even then i am still day dreaming and getting tired easily while working out. I had these issues since my childhood on a very small scale but from past 6 months it's been aggravating. What could be problem?

Brief Answer:
About gynecomastia:

Detailed Answer:
Hello and thank you for asking!
I have gone carefully through your concern and seen the attached results
.There may be three hormonal dis balances causing your symptoms;
-Hyperprolactinemia (high prolactine level)
-Hypothyroidism ( under active thyroid)
-Hypogonadism ( a low testosterone level).
So these are the three most common pathologies that should be excluded/ confirmed at first.
So you need to check prolactine, TSH and testosterone.
Hope the information will help.
Best wishes.

My testosterone and prolactine levels were normal. I started observing gynecomastia and then tummy fat and belly fat two years back, I don't eat large quantities either and I added 14% body fat from last two years. I feel week, less energetic, depressed. I am really struggling to concentrate and memory is taking a toll.

Brief Answer:
No hormonal disbalances;

Detailed Answer:
Hi again,
I do appologise,in the second attached sheet of the results there is detailed thyroid function checked.
There is a large hormonal panel and everything is normal.
There is no hormonal disbalance for weight gaining, gynecomastia or psychological changes you have noticed.
I would suggest to continue with physical activity, weight loss.
There is no need for further exams.
Best wishes,
Dr.Mirjeta
