Question: I am a 34 year old height and active male. Last year, after gross hematuria and severe waves of pain, I had a CT scan that found a 5mm+ stone in my right ureter which caused on and off pain until it was passed nearly 2 months later. Although the scan didn't detect them, I had passed multiple stones prior to the biggest one. One night there was a very sharp pain (like a needle stabbing it) at the tip of my penis. The next morning, I needed to urinate but nothing came out and then a speck of black debris came out followed by the largest stone and then another black speck of debris and then finished my steam of urine. The large stone had several VERY sharp points that even possible my finger and caused bleeding when I picked it up. It was unbelievable how sharp the stone was. It reminds me of a sandspur where every point is like a needle. Since then, there has been many small black "specks", similar to ground black pepper or perhaps very small poopy seeds, that are visible in my urine every now and then. Occasionally, I hesitate to begin urination and the flow sometimes stops and then continues. Every now and then, I dribble urine at the end. More often than I wish, after urinating, I have the sensation of warmth in my penis and then semen flows out without orgasm or arousal.

I am curious to know if the passing of a kidney stone can effect or irritate the prostate in any way. When I mentioned the sharp, needle-like sensation at the tip of my penis before passing the stone, I have this thought that maybe the sharp stone got lodged in my urethra in the prostate (causing the tip of penis pain) and then the next day somehow it was dislodged and then finally passed through penis.

Also, can symptoms of any enlarged prostate come and go over time or would it be an ongoing issue? Furthermore, I also don't "feel" the urge to urinate, but somehow mentally become aware of the need to urinate and then do so. This, however, was noticed by myself a couple years ago. I do wake up and urinate at night more often than I would like and wake up with a stiff lower back in the morning and have a hard time sleeping. Every few weeks or so, I have a sudden, very brief (one second) jolt of pain originating in my upper back area. Any information regarding the concerns I have expressed will be appreciated.

Based o the facts that you have narrated you seem to have tendency to form urinary stones repeatedly which needs to be investigated thoroughly so as to take necessary precautions and modify dietary measures to avoid recurrence of stone formation.

Consult qualified Urologist for clinical examination and get following basic tests done to establish cause for stone formation .
1) Urine routine and Urine culture
2) Ultrasound Scanning of abdomen
3) Blood tests for -
a)Serum Calcium
b)Serum Phosphate
c) Parathormone study
d) Stone Analysis (You will need to collect the stones that
you pass through urine and submit them to lab for analysis
to know the composition of stone .
4) Renal function tests .

All the symptoms that you have are due to irritation of prostatic part and entire urethra caused by spiky stone.

Further treatment and dietary regimen will depend upon the results of these tests and composition of stone detected on stone analysis so the it will help to avoid recurrence of stone formation ..
