Question: I am a 45 and nine months female in excellent health. I would like to conceive and am actively trying during what should be my fertile week and I understand the potential risks and complications associated with my age. I also understand it may be difficult or impossible as my egg quantities and qualities have declined. My period has been irregular this past year suggesting peri menopause. Cycles have ranged from 19-45 days, averaging 26 days. I live in a small jungle village with no gynecologist and I decided on my own to try oral 200mg progesterone in an effort to assist my body. Now I am on day 33 and the period tracker app says I am three days late but as I said I have been irregular. I have taken two pregnancy tests two days apart both negative. I don’t know what to do now do I stop the progesterone or continue? I am also considering to ask for clomid but first the progesterone....can you pls advise me

At 33 days when there is no pregnancy, there is no use of continuing with progesterone support. You can stop it so that your periods can start. At this age, you need to get your Serum AMH levels evaluated to assess the ovarian reserve that you have. In case the AMH levels are low then the chances of conception are also less.
Before starting with any medications on your own it is best to consult a gynecologist first and get a few test done so that your periods can be regularized. Then ovulation inducing medicines like Clomiphene can help you.

Thank you, I would like to see a gynecologist but I don’t have access to one at the moment so that is why I have consulted with you
I would like to regulate my period and then begin the clomid. Will you pls advise me how best to do so using the progesterone or any natural means. I will ask the clinic here for a blood test and see if I can get that. Then I can ask for the clomid, will you pls tell me when to begin that and how much to take?
I agree it would be best to see a gynecologist but at the moment I am unable to do so and I would like to try these methods

The regulation of periods depends on your period frequency and timings. You need to start with Metformin and try to lose some weight in order to regularize the periods.

You can take progesterone in case the periods are delayed to get a withdrawal bleeding. Otherwise, progesterone is not needed routinely.

Clomid is started on Day 2 of the cycle and continued till day 5. Then fertility monitor can be used to track the time of ovulation and have intercourse during that time.

I am unsure why metformin is suggested as I am very thin 135lbs 5’11”

I will s/c the progesterone and when should I expect my period to begin?
I have read that progesterone helps lengthen a shortened literal phase and supports conception and implantation and decreases chance of miscarriage...?


Taking clomid days 2-5 means while I am on my period then, correct?

Metformin is a standard treatment of PCOS as it helps in decreasing the metabolic imbalance. Other medicines like Myo-inositol are also helpful.
After you stop taking progesterone the periods are expected after 3-4 days. Progesterone helps in lengthening the period and supports endometrium but is recommended only if you are deficient in progesterone which can be checked with the help of a blood test.
Clomid is taken while you are having the period considering the day the periods start as day 1.
