Question: I am a diabetic. Type II - insulin dependent. The past few days my afternoon sugars have been lower than normal (between 44 and 90). I have decreased my Humalog and eaten a few extra cards. I am at 96 just now and have the strongest smell of acetone on my breath. I see my endocrinologist tomorrow. Should I be concerned between now and then? I feel ok except for being a bit dizzy

I have gone through your concerns carefully.

Ketosis with normal blood sugar and without other symptoms ( anorexia from many days, fever, diarrhea etc) in type 2 Diabetic patient can be caused by inappropriate carbohydrate intake.

As a result, your body must use fat as fuel and ketones are increased manifested by the fruity smell on the breath.

If you have a urine deepstick you can check ketones in urine at home otherwise try to eat more carbs today, drink plenty of water and keep monitoring blood sugar.

In type 2 Diabetic patient, ketoacidosis is a rare condition with normal blood sugar and without other symptoms.

So, I'd suggest you can see your physician tomorrow and explain the situation.
