Question: I am a male, 67 years old, had my prostate seeded @8 yrs ago. During ejaculation, I experience sharp pain on both sides of my torso which feel like pulling from my kidneys. The pain persists until the orgasm completes and occasionally have abdominal cramps afterwards. I have no pain during urination and no discharge from the penis.

Based on the facts that you have posted the pain or discomfort that you get on both sides of the trunk during ejaculation could be explained as detailed below.

During ejaculation thick semen is delivered in the urethra through the ejaculatory duct and prostatic secretions through multiple ducts of the prostate on either side of the opening of ejaculatory duct.

As you have prostate seeded 8 years ago it is likely that the opening of these ducts and ejaculatory ducts are partially blocked due to scarring leading to obstruction to free flow of semen and prostatic secretions into the urethra which in its turn leads to increase in tension in seminal vesicle and testicles (Through back pressure).

Since nerve innervation of testicles and other genital organs like seminal vesicle, the prostate is commonly by Genito Femoral nerve with kidneys which originates at the level of the trunk, the discomfort and pain caused by increased tension in testicles and seminal vesicle are felt along with the entire distribution of Genito Femoral nerve (i.e over the trunk). This is called as referred pain.

Taking pain killer like Brufen or Diclofenac one hour before planned sex can help to reduce the intensity of pain.
