I have gone through your details and here is my opinion.
Commonest cause of fatigue and exhaustion in non diabetics is multiple nutritional deficiencies which includes a deficiency of Vitamin B12 ,folic acid and iron.This causes pallor ,anaemia and extreme tiredness.This gets revealed by blood work -Haemoglobin,serum iron levels,total iron binding capacity (TIBC),Haematocrit (PCV)serum vitamin B12 levels and folate levels.
Thyroid gland secretes hormones which are involved extensively in body metabolism.Hypothyroidism also cause such symptoms and should be ruled out by checking serum thyroid levels (T3,T4and TSH).
You need a proper antibiotic course for your tympanic perforation and ear infection.
Keep yourself well hydrated and take proper amount of salt in diet.Take foods rich in vitamin B 12 like milk,cheese,eggs,fortified breakfast cereals,lean meat and poultry.This would relieve your leg cramps.
You need to consult your clinician for evaluation and further work up.
Kindly have high fibre diet with plentiful fruits and raw vegetables,take plentiful proteins,have commercially available vitamin supplements,be physically active,de stress yourself,avoid alcohol and caffeine and don't be sleep deprived.
Discuss an alternative of Beclofen with your doctor as it's most common side effect is tiredness.
