Brief Answer:
Eczema, dermatitis or fybromyalgia.

Detailed Answer:
Hello dear, welcome to Ask a doctor service.
I read your query and here is my advice.

You have to do the mammography and ultrasound of the breasts in order to check for beast cancer (padget disease can cause pain).
If mammography, ultrasound and biopsy come out negative then the risk of malignancy is eliminated and we can think about other causes such eczema, dermatitis or even fybromyalgia.
In case of eczema and dermatitis the skin if nipple is red, irritated with flaky patches.
If the skin is normal then it can be fybromyalgia which causes mussels and tissue to be tender and sore and cause pain.

Hope I have answered the question.
Let me know if I can assist you further.
