Question: I am male 49 , I have mild high temperature (fever) , facial flushing , headache at both sides of head , back neck pain left side , lower back pain left side gets worse when moving . I am hypertension and taking medicine for the same . I don't think these symptoms related to hypertension . Cause I have Bern hypertension for the past 20 years . I did some blood test and abdominal ultrasound . What could be the problem ? . I have seen some internal doctors but they seem to look for something else rather than the real problem . They have been blaming these symptoms for the blood pressure . They keep changing the medicine and thus the blood pressure is getting up and down . I am sure it is not the blood pressure cause I have been living with this for ages now . Any suggestion ? I wish I can eliminate other causes on order to rest and sure it is blood pressure related problem . CT scan of the brain is ok . Thyroid ,t3 ,t4 levels ok .

Hello!

Welcome to Ask a Doctor service!

I passed carefully through your question and reviewed all your uploaded tests and would explain that there are a few points of concern in your blood lab tests:

- Your Hemoglobin levels are high, possibly indicating polycythemia
- Very low vitamin D
- High ferritine levels.

Coming to this point, I would recommend checking also dopamine plasma levels ( in order to investigate or possible Page syndrome) and urinary metanephrines ( to investigate for possible pheochromocytoma).

Consulting with a hematologist is necessary, considering the high Hematocrit and Hemoglobin levels, in order to distinguish the type of Polycythemia (vera or secondary). Inflammation tests (PCR, fibrinogen levels, ESR, etc.) should be checked too.

You should also discuss about high ferritin levels and possible hemochromatosis. In this regard, a cardiac MRI would be helpful.

Hope to have been helpful!
